Telemedicine is a way to connect with healthcare providers via virtually. You can talk to your provider from any place, including your home. You don’t need to go to a clinic or hospital.
The patient accepts that he/she needs access to a tablet or smartphone (can be android or ios) and a good internet connection in order to have an efficient telemedicine appointment. First, you sign up for the application. Then you can select a provider. At the time of your appointment, you talk to the provider using a phone or a tablet. You use video so you and your provider can see each other. If the internet connection isn’t strong enough you can use the audio option as well. HabariDOC allows a secure transmission of medical information between you and your healthcare provider.

We will not record video or audio of the visits with your provider. The medical information related to the history, records, and tests of the patient will be discussed during the telemedicine appointment via video and audio. Patients' medical information obtained including diagnostic tests and prescriptions will be stored on our secure server. Only physicians that have had a consultation or appointment with the specific patient will have access to the information. Patients also have complete access to their records via the patient portal.
If there are people around you during the audio/video consultation session, they may hear some of the information shared between you and your provider. You should be in a private place, so other people may not hear your personal information. Your provider will tell you if someone else from their office can hear or see you.
We use telemedicine technology that is designed with optimal precaution to protect your privacy. If you use the Internet for telemedicine, use a network that is private and secure.
There is a very small chance that someone could use a technology to hear or see your information in case of a hack, in which case, HabariDOC will inform you of any risk related to the security of your personal health information.
The username and password you use on the app are specific to you. Protection of this information is the responsibility of the client.
If you want an office visit after a virtual consultation, you and your physician will have to agree on the location and the healthcare facility where they wish to see you. You will be responsible for such scheduled appointments and related fees.
- You can stop using telemedicine at any time, even during a telemedicine visit.
- Call 9873 and say you want to stop, and we will deactivate your account.
- N.B. All the information that's already on the server will be kept for your own convenience, security, and legal purposes.
- What you pay depends on the physician's appointment fee displayed on their profiles.
- If your provider decides you need an office visit in addition to your telemedicine visit, you may be required to pay for both visits.
- Only sign this document if you want to use HabariDOC telemedicine.
- The patient can withdraw his/her consent at any time by simply terminating the use of HabariDOC.

You understand that all the laws that are protecting your privacy of medical history or information are also applied to telemedicine practices
You understand that you can withdraw the consent at any time and that it will not affect any of your future treatment.
You authorize health care professionals and use telemedicine for your treatment and diagnosis.

You agree that this agreement is legally enforced in Ethiopia. Unless specified otherwise this agreement as well as other related agreements will be governed by Ethiopian law.
